Below is a sample of the online Registration Form which must be completed annually by each data controller. The sample should assist prospective data controllers in compiling the information required to complete the online form. Please note that the OIC does NOT accept manual/paper applications. All applications can only be submitted via the online registration portal, where the data controller's registration particulars must be entered on the platform. The form CANNOT be completed, scanned and uploaded. The option to submit an application is ONLY provided AFTER the required information has been entered and documents (including proof of payment) are uploaded.
