Below is a sample of the online form to be used in completing your application for registration as a data controller pursuant to sections 15 and 16 of the Data Protection Act, 2020. It is hoped that this sample will allow for a more fulsome preparation of the information and any supporting documentation that will be required for entry and/or uploading during the online process. Please be reminded that the registration process is fully online, and the OIC does NOT accept manual/paper applications. All applications can only be submitted via the online registration portal, and the data controller's registration particulars must be entered on the platform. The form CANNOT be completed, scanned and uploaded. The option to submit an application is ONLY available to prospective registrants AFTER the required information has been entered and documents (including proof of payment) have been uploaded.
